Here’s the order in which the patients in the hospital are killed (except Laura):

1. Paul is forced to face his fear of darkness when the Boogeyman turns off all the lights in the basement as Paul goes there to smoke secretly. Since he’s so frantic in his attempt to escape with the help of an elevator, his body is cut into two.

2. Mark, the hypochondriac, is dead by drinking his self-made super power cleansing liquid. Why? Because the Boogeyman puts roaches inside his bag of chips, so when he eats the chips, he accidentally puts one roach inside his mouth. Being a clean freak, he wants to clean his mouth and the Boogeyman gives him the cleansing liquid.

3. Alison, the cutter, is drugged and strapped on a bed. The Boogeyman has “injected” maggots under her skin and he gives her a knife so when she gains consciousness, she freaks out and starts cutting the maggots out from her arms. That’s how she dies.

4. Darren’s heart is cut open with a tool and the Boogeyman then rips out his heart.

5. Nicky’s strapped onto a hospital bed with many tubes all over her body. A machine is running and sending fluid inside her body so that her body becomes more and more swollen and it “explodes”.

So who’s the Boogeyman? It’s actually Henry Porter, Laura’s own brother, who’s become “crazy” during the time he was treated by Dr. Mitchell. He forced Henry to get inside a walk-in closet, locked the door, and told him to face his fear. He freaked out (definitely!) and then he thought to himself that the Boogeyman had no fear, so after he was released from the hospital, he put on a mask and started forcing the others to face their fears. Dr. Jessica Ryan is one of his victims and he turns her into a crazy woman.

The patients can’t go out of the hospital since the phone doesn’t work and the computer is broken, so they can’t open the door to outside world. In the end, Henry walks out unscathed. He puts the Boogeyman’s mask on Dr. Jessica’s face and tells her to walks towards Laura. The scared Laura stabs her neck (thinking that the masked person is Henry) and the police barge in. Finding nobody else alive except Laura, they take off the mask and find Dr. Jessica Ryan dead. Laura can’t believe her eyes. She keeps on telling the police that Henry has gone mad and he’s still out there, but by now the police suspect her as the killer of all those people. So Laura can’t do anything but scream while the police take her. The end.
